Rhinoplasty is one of the most challenging aesthetic surgeries that require the most careful planning and highest level of experience. Due to factors such as the complexity of the nasal anatomy and the structure of the skin specific to the patient, weakness in the cartilage structure, and the steps taken in the recovery process, respiratory problems or nasal deformities may develop after rhinoplasty, even if it is performed by the most experienced and skilled specialists. In such a case, the need may arise for revision rhinoplasty, also called secondary rhinoplasty, in order to correct the results of primary rhinoplasty.

What is Revision Rhinoplasty? How Is It Performed?

Revision rhinoplasty is the nasal aesthetic surgery performed secondary to the problems in nasal functions and shape that develop after rhinoplasty surgery, or due to the unmet expectations of the patient. Revision rhinoplasty, which can be performed with open or closed technique, can be performed under general anesthesia, local anesthesia or intravenous sedation. General anesthesia is generally preferred for patient comfort. The choice of technique is determined according to the scope of changes to be made in the nose, the needs and expectations of the patient. Revision rhinoplasty may take longer than primary surgeries for reasons such as excessive deformities and the need for cartilage to correct the shape of the nose. 

Who Can Have Revision Rhinoplasty?

Revision rhinoplasty may be considered due to cosmetic reasons or experienced functional problems. In order for the procedure to be performed, the shape of the nose must be fully achieved after the primary procedure, and the recovery process must be completed. The necessity of waiting for  12 months in case of an unexpected situation can be flexible at the decision of the surgeons. Nonetheless, patients should not act quickly in this case, unless there is any obligation. The conditions that lead to the need for revision rhinoplasty can be listed as follows:

Collapsed nasal septum,
  • Insufficiently configured the tip of the nose, being too wide, too narrow, too low or too lifted.
  • The nasal ridge being not smooth enough or too curved,
  • Development or continuation of asymmetry problem in the nostrils,
  • Unnatural look of the nose due to its structure,
  • The development of respiratory problems after primary rhinoplasty, or the inability to resolve a pre-existing nasal congestion.

Planning Process Before Revision Rhinoplasty

Performing a complete evaluation process before revision rhinoplasty is the most important requirement for both eliminating functional problems and meeting the expectations of the patients. In this context, the nose is analyzed in detail, evaluating the thickness, integrity and mobility of subcutaneous tissue. The areas that have not been attended in the primary procedure, or the presence of a need for grafts should be examined. The process is not finalized without examining the nasal ridge, nasal septum and cartilage profile. In addition, all functions of the nose are also evaluated. 

Recovery Process After Revision Rhinoplasty

After the general medical examinations are completed, discharge procedures can be performed. Moderate to mild pain may develop based on the scope of post-operative changes. The resulting pain can be controlled with painkillers. The recovery process may also vary according to the scope of the procedures performed during revision rhinoplasty as well as patient-specific factors. The points to be considered after the procedure are generally the same as the points recommended after primary rhinoplasty.
